OFFICE POLICIES
CANCELLATIONS
Please provide adequate notice (48 business hours’ notice for new patient appointments and 24 business hours’ notice for follow up appointments) if you need to cancel or change an appointment. We often have a waiting list and, because we do not double book, your appointment time cannot be used by anyone else if you cancel on short notice or do not show up.
MISSED APPOINTMENT FEE
Cancellations without adequate notice or no-shows will be subject to the full fee that would have been charged for the scheduled service. You will be charged the FULL FEE for your appointment ($75.00 for follow up acupuncture appointments, $100.00 for new patient acupuncture appointments, $100 for SAAT follow up appointments, or $200 for SAAT new patient appointments) if you do not provide adequate notice (described above) for appointment changes. This policy will be enforced without exception.
DEPOSIT REFUND POLICY
Deposits will only be refunded in the event that a new patient appointment is cancelled with adequate notice (48 business hours). Requests for deposit refunds must be made within 10 days of the cancelled appointment.
CARD ON FILE POLICY
We require a valid credit card on file (not an HSA card) in order to schedule appointments. This card will be charged missed appointment fees, returned check fees, and any other balance owed. Future appointments will not be scheduled until any outstanding balance is paid in full.
FINANCIAL POLICY
Payment is due at the time of service. We accept cash, MasterCard, Visa, American Express, Discover, personal checks, and most Flexible Savings and Health Savings Accounts. Any fees assessed to our clinic from checks that are returned due to insufficient funds must be promptly paid, along with the original amount of the check. We do not offer refunds for services once treatment has been rendered. All sales of herbs, topical medications, supplements, and other items are final. A $30 fee will be charged on checks returned for non-sufficient funds.
CREDIT/DEBIT/HSA CARD SURCHARGE POLICY
Fees for services paid with a credit, debit, or HSA card will incur a 5% surcharge. You can avoid this fee by paying with check or cash.
INSURANCE POLICY
We do not participate in any insurance billing and full payment is due on the day of your visit. If you have insurance that pays for acupuncture, you are responsible for all aspects of filing your own insurance claims and your responsibility for payment to this office is not contingent upon reimbursement from your insurance company. You are responsible for verifying that your insurance policy allows you to file your own claims with a superbill. We do not provide W-9s to insurance companies, nor do we make phone calls to insurance companies or provide documentation beyond receipts. Fees will not be negotiated or reduced, even if your insurance company’s allowable amount for acupuncture services is less than you have paid.
FRAGRANCE POLICY
Many of our patients are extremely sensitive to perfumes and other odors. For this reason, we ask that you refrain from applying perfume, cologne, essential oils, or other strongly scented products on the day of your treatment. In addition, we ask that you be mindful of odors that may linger on your clothing, including pet odors and cigarette or cannabis smoke.
